What to Bring

• Photo ID
• Referral slip from your general dentist. If you do not have a referral, we can still see you! Call us at (540) 408-0020 to discuss if an endodontist is the right specialist to help you.
• New patient registration forms. We will send you the forms electronically so you can complete them before your first visit. Filling these out before your appointment helps ensure we can maximize your time with the doctor. If you are not able to complete them in advance, please allow yourself additional time to complete forms in our office before your appointment.
• List of medications and medical conditions. Please bring an up-to-date list of your current medications and medical conditions.
• Dental insurance card(s). Please bring your dental insurance card or a photograph of it with you to the appointment.

Tell Us Before Your Appointment

• You are pregnant
• Require antibiotics prior to dental procedures
• Have dental anxiety requiring medication prior to treatment

Please note:

• due to the size of our treatment rooms, patients will only be able to bring one guest with them to appointments. For patients under the age of 18, a parent or legal guardian must accompany them.
